Dear Parent(s)/Guardian:
The Board of Education urges parents to schedule, to the extent possible,
medication of a student outside of school hours. If medication, including
over-the-counter drugs, is to be dispensed during the school
day, it needs to be done in accordance with the following Stow-Munroe
Falls Board of Education policy:

A request completed and signed by your physician
must be on file

A medication release form must be signed by the parent/guardian
and be on file in the school office

Medication such as aspirin or
Tylenol will not be administered under any circumstances by school
personnel unless prescribed by a physician.
Cough
drops are considered medication.

New request forms shall be submitted each school year.

Parents/guardians shall agree to:
-Assume responsibility for safe delivery of medication to
school
-Notify school if they change physician
-Submit a revised request signed by the physician who prescribed
the medication
-Notify school if any of the information provided by the physician
changes, including dosage.

Medication shall be received in the container in which it
was dispensed by the pharmacist and labeled with:
-Student’s name
-Name of medication
-Dosage
-Time of administration
-Licensed pharmacist’s name
*Ask the pharmacist for a separate container for school
with a label as stated above. The pharmacist will often
divide
the prescription
for you.

Over-the-counter medication must be in the original
container labeled with the child’s name,
dosage, and time(s) to take the medication.
At the top of this page, you will find a copy of the forms needed.
Many parents take the physician’s form with them to the physician’s
appointment in the event a medication is prescribed. This can
save you time and
assure your child receives the medication as prescribed.
REMEMBER: NO MEDICATION CAN BE ADMINISTERED UNLESS THESE FORMS
ARE SUBMITTED TO THE SCHOOL OFFICE.
